Reporting a near miss, unsafe act or condition

At Liverpool Hope University, we are committed to providing a safe, healthy and supportive environment for all students, staff and visitors.

One of the most effective ways to prevent accidents and improve safety across campus is through early reporting of:

  • Near misses – events that almost caused harm
  • Unsafe acts – behaviours that increase the risk of injury
  • Unsafe conditions – physical hazards in the University environment

Reporting these issues helps us fix problems before anyone gets hurt.

Why reporting matters

Every report, no matter how small, helps the University. Reporting can:

  • Prevent accidents by identifying hazards early

Examples of what to report

You should report any situation that could cause harm, including:

  • Spills, leaks or wet floors without signage
  • Damaged flooring, uneven surfaces or blocked walkways
  • Unsafe behaviour such as incorrect use of equipment
  • Faulty electrical equipment or obstructed fire exits
  • A near miss where an injury almost occurred

What to include in your report

To help us respond effectively, please include:

  • Location (building, floor, room or area)
  • Date and time of the event
  • What you observed
  • Any immediate action taken
  • Your contact details (optional, but helpful for follow-up)

What happens next?

Once your report is submitted:

  • The Health and Safety Advisor review the information
  • An assessment is completed to determine the level of risk
  • Any necessary corrective actions are arranged
  • If you provided contact details, you may receive an update or request for clarification

All reports are handled confidentially and used to improve safety, not to assign blame.
